UCSF Health provides care to patients across the multiple UCSF campuses and its affiliates.
The Pharmacy & Therapeutics Committee (P&T) is a medical staff committee that reports to the Executive Medical Board. The Committee receives input from medicine, nursing, pharmacy, quality improvement, revenue cycle, contracting, and others. P&T is responsible for maintaining, updating, and administering the medication formulary. The P&T committee also reviews and approves policies, procedures, guidelines, and protocols that involve the use of medications and biologics including cellular-based and gene therapies. Importantly, the Committee also oversees the effective use of health care resources in order to minimize overall pharmaceutical costs, improve patient access to more affordable care and provide an improved quality of life.
How to submit a request for P&T consideration
To request an item for P&T consideration, select the appropriate page that best corresponds to your request:

Once you have reviewed and completed the requested materials, please contact the medication formulary manager to reserve a place on an upcoming P&T agenda.
The UCSF Health Pharmacy & Therapeutics Committee meets once monthly on the second Wednesday of every month from 12:05 pm - 1:30 pm.
A preparatory meeting, Pre-P&T, is held on the third Tuesday of every month between 2 pm - 4:30 pm. The purpose is to review and vet all materials going before the P&T Committee.
