Formulary Extensions & Changes to Existing Formulary Medications

Occasionally, there may be a need for the P&T Committee to review and approve select formulations of a medication already on the formulary. The review serves as a way to document the change, assess the financial impact, and inform clinical services of the update. 

Requests for formulary extensions include but are not limited to any of the following: 

  • Continuation of outpatient therapy or the addition of a high-volume, non-formulary medication that is used extensively but has not been added to the formulary 
  • Addition of a new formulation of an existing formulary medication (e.g., ointment requested in place of cream, oral suspension in place of oral tablets) 
  • New indication to an existing formulary medication (e.g., request for expanded use for a new indication of an existing formulary medication) 
  • Removal of an existing formulary medication (e.g., removal of a medication that is no longer available, no longer used, or is no longer the standard of care) 
  • Change to ordering tools that impact medication selection (e.g., modification of an order set that impacts medication selection) 
  • Request a change to formulary status (restriction-related, or other) based on new data/real world-evidence or similar of an existing formulary medication (e.g., restricting use of a formulary medication to a specific service or patient population based on new information)

Submit a formulary modification proposal

To submit a formulary modification proposal, please use the Formulary Extension or Modification template. The key elements to include: 

  • Champion or designee: Name, title, contact information
  • Sponsoring body [committee, group, service, department, etc.]
  • Background or statement of problem
  • Supporting evidence and data
  • The requested action [e.g. vote to approve] with the exact recommended action statement
  • The committees, services, individuals, etc. who have reviewed and approved the recommendations thus far

Review and approval process 

These requests are presented during the recurring Pre-P&T Committee meeting live. If approved, the request would be placed on the P&T Committee Consent Agenda. This means that the P&T Committee members would receive the background and recommendation via email ahead of the meeting without a live presentation. During the P&T Committee meeting, committee members may ask for clarification or follow-up. If there are no questions, the P&T Committee will vote privately and you will be notified of the outcome within 10 days of the P&T Committee meeting vote.
