Committee Reports

Pharmacy & Therapeutics Subcommittees 

The Chair of the Pharmacy & Therapeutics (P&T) Committee appoints subcommittees to accomplish the work of the Committee. The subcommittees report their accomplishments, recommendations, challenges, or future areas of work to the full Committee based on an agreed-upon timeline. 

UCSF Health P&T subcommittees 

  • Antimicrobial 
  • Antithrombotic and Hemostasis  
  • Dietary/Nutrition Services (requests are ad hoc)  
  • Medication Safety 
  • Medication Value Analysis
  • Oncology 
  • Pediatrics 
  • Pharmacogenomics 

Each subcommittee is required to present a summary of their work to the P&T Committee based on an annual, quarterly or more frequent basis depending on the subcommittees work. 

All subcommittee reports should provide the P&T Committee the following components using the Subcommittee Report PowerPoint template.

  • Champion or their designee 
  • Presenting on behalf of [committee, group, service, department, etc.] 
  • Subcommittee priorities 
  • Does the work of the subcommittee apply to adults or to pediatrics or both 
  • Would the work or findings identified by the subcommittee be applied across UCSF Health or only at certain campus locations (e.g., UCSF Emergency Dept., Mount Zion, Mission Bay, Parnassus, Benioff Childrens’ Hospital SF, Benioff Childrens’ Hospital Oakland, etc.) 
  • Data/supporting material for each priority targeted 
  • The requested action; vote to adopt the subcommittee’s report and any future areas to target 
  • The committees, services, departments, individuals that have contributed to the subcommittee’s work  

Review Process 

These reports are presented during the recurring Pre-P&T Committee meeting and on the P&T Committee agenda with a live presentation. During the P&T Committee meeting, committee members may ask for clarification or follow-up. If there are no questions, the P&T Committee will vote to adopt the subcommittee’s report.
